CERTIFICATE of ACCREDITATION
This certificate will be issued to labs that meet the standards of an accreditation program approved by CMS.
This would include all programs - State and Private - that have received "deemed status" under CLIA.
Fees for this certificate would be determined by the issuing agency. COLA has received "deemed status".
HOW TO GET A CERTIFICATE:
To apply for certification, labs must provide information to CMS. This is done by filling out the #116 form.
This forms may be sent to you by calling your local CMS office. It is also available from most State CMS office web sites.
This information will determine classification for fee payment for government inspection.
If you will be inspected by COLA there is a fixed fee schedule. A lab must notify CMS regarding changes in ownership,
lab name, location or lab director.
